  1. Date, time and location of the problem

  2. The computer name or asset tag if college asset

  3. The username which was used to log in while the problem was occurring

  4. What was happening before the problem started (i.e. running a video when the projector went blank or changing display settings or trying to sign in, etc.)

  5. What happened when each of the steps above were attempted

  6. When will you be available to help us test a solution if we’re unable to get to the room before the current class ends
